I calculated calibration factor of EYA Optical Lever.
L : distance from mirror to QPD.(~1130mm)
δ : angle of mirror(Pitch or Yaw)
C1 : This factor transform output of QPD into displacement (mm) (http://klog.icrr.u-tokyo.ac.jp/osl/?r=1360)
C2 : caliblation factor. C2 = 1/(8*L*C1)  
Result
C2_Pitch = 109.5
C2_Yaw = -103.2
next step, to check this caliblation I used coil-magnet actuator.
I attach this result graphs.
H1,H3-Actuator : Pitch direction
H2,H4-Actuator : Yaw direction
Horizontal axis : Input voltage of actuator
Vertical axis : mirror angle
According to the calculation using 4 actuator efficiency : 3.3um/V
gradiation 16.5 urad/V, but in this test the Pitch is less than 16.5 urad/V, and the Yaw is
larger than 16.5 urad/V.
We should check again.
